Financial Information

At Erickson Oral and Maxillofacial Surgery, we are committed to making your treatment successful. Please understand that payment for your service is considered a part of your treatment. The clear understanding of our financial policy is important to our professional relationship.

Insurance is a contract between you and your insurance company. We are not a party to this contract unless you have a managed care plan that we participate in. You are responsible for the timely payment of your account.

Insurance

As a courtesy to our patients and families, we submit all claims to your primary insurance company and accept assignment of insurance benefits if we are in your insurance plan. If your insurance does not pay the full balance within 45 days, the balance of your account will become your responsibility. Please be advised that some (and perhaps all) of the services we render may be considered “non-covered” by your insurance company. In this case, they are not considered under your medical/dental insurance plan. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Please be advised that your insurances “usual and customary” fees may not be the same fees of Erickson Oral and Maxillofacial Surgery, they establish “usual and customary” fees based upon the contact with your employer. In some cases these amounts may be less than our actual fees. The difference will become your responsibility.

Please contact your medical /dental plan prior to any procedure to check the benefits/eligibility. If we are able to predetermine insurance coverage, you may know your approximate financial responsibility prior to surgery. If not, 50% of the total surgery/ anesthesia fee is required prior to treatment.

Co-pays

Families with managed care must pay their co-pays at the time services are rendered as stated in your benefits plan, if we are a provider for your plan. If not payment for the service will be required at the time of treatment.

Pre-estimate for Dental Coverage

If you would like a pre-estimate sent to your insurance company prior to the procedure, we would be more than willing to do so, however, this process typically takes between 3-4 weeks to be completed.

Payments

We accept cash, check, Visa, MasterCard, and Discover. Please ask us about Care Credit Financing.
